1. Are your services covered by Medical Insurance? In order to keep our rates as low as possible for everyone, we require cash at time of services. We will gladly complete a standardized SuperBill, with insurance-industry mandated Diagnosis and Procedural Codes so that you may obtain appropriate reimbursement for your payment.
We can also refer you to an independent agency to assist you in maximizing your insurance reimbursement.
2. Where are you located? We are located at 3121 Park Avenue, Suite D, Soquel, California 95073.
3. Directions to Clinic: If approaching from the North, travel south on Hiway 1 past Santa Cruz and Capitola, exit Park Avenue, turn left on Park Avenue (away from ocean), to cross street of Soquel Drive. Cross through the intersection, we are in the large green commercial building behind the gas station.
If approaching from the South, travel north on Hiway 1 past Watsonville, Rio del Mar, exit Park Avenue, turn left on Park Avenue (away from ocean), to cross street of Soquel Drive. Cross through the intersection, we are in the large green commercial building behind the gas station.
If approaching from East, travel west on Hiway 17, take the ‘fish-hook’ exit to Hiway 1 South, past Santa Cruz and Capitola, exit Park Avenue, turn left on Park Avenue (away from ocean), to cross street of Soquel Drive. Cross through the intersection, we are in the large green commercial building behind the gas station.
If approaching from the West, dock your boat in the Santa Cruz Harbor, ask the Harbor Master to call a taxi, give them the address of 3121 Park Avenue, Suite D, Soquel. We’ll help you get the barnacles off.
4. Lodging—our out of town, out of state, and international clients enjoy staying at the Land of the Medicine Buddha, a Tibetian Buddhist retreat center located a short distance from our Center. Other hotels, resorts, and Bed and Breakfasts are available locally. Call our front desk staff at 831.462.3776; they will help you with suggestions to meet your needs and budget.
5. Hours—typically Monday-Fridays, 9:00am-5:00pm PST.
6. Fees—New Patient appointment, 1 hour 30 minutes, $420. Returning Patient appointment, 1 hour, $375. Shorter appointments for specific complaints are billed in quarter hour increments of $95.
We have a number of Programs that offer discounts, both in-Clinic and our OnLine TeleMedicine Programs.
7. What is your ‘typical patient‘? Our patients are of all ages, all walks of life, all socioeconomic levels. Our office staff speaks English, Spanish, Hindi, and German.
8. Does Dr. Tom perform House Calls? Yes, he does. Whether a house call within the local community, or accompanying a patient to a foreign country for diagnostics or treatment, if it’s important to you, Dr. Tom listens!
