A calendar invitation has been sent to your email address.

You will receive an confirmation email with New Patient Intake forms which must be completed and returned by email or fax 24 hours before your Consultation.

We will not be able to consult with you until the forms are in our hands.

If you need to reschedule your appointment, please go to the calendar invitations to change your appointment. There is also a link in the confirmation email to change your appointment.

Unfortunately, due to the constraints of the digital system, you may need to enter your credit card info for a second payment when you reschedule. We promise to refund any extraneous charge immediately after your consult.

Please do not call our office as our phone lines are already swamped. DO email the address in your confirmational email if you are having problems.
